- the referral;
- passport or ID card and the residence record;
- results of tests not older than 10 days: blood count, urine, blood sugar, ECG, chest X-ray (fluorography) not older than one year;
- the voluntary insurance policy, if you have one;
- your regular medicines (patients on insulin: a supply for three days), slippers, a cup, spoon, toothbrush, towel.
Planned operations and some examinations need a co-payment by the price list. All payments are made only at the cash desk (Building A, ground floor, and Diagnostic Center window 6) against a receipt. Payments to staff are prohibited. Report any demand for money to the chief physician (tel. +996-39-2-26-10-01) or to the hotline of the Ministry of Health, +996-39-2-26-41-00.
Patients from the districts and from beyond the line
Patients from the district hospitals are transferred by agreement of the doctors. Emergency care is given to everyone; for planned treatment a residence record of the Zone is required.
Discharge
Patients are discharged on working days after 12:00. You receive the extract from the medical history (free) and, if you work, the certificate of incapacity for work. A duplicate extract is issued by the medical records office, Building A, room 014, within 10 working days (paid).
